Testimonial Consent Terms

By clicking on the “I consent” button, I grant Jackson Hewitt Tax Service Inc. and its affiliates (collectively, “Jackson Hewitt”) and any of Jackson Hewitt’s licensees (collectively, “Licensees”), the perpetual, worldwide right and permission to exhibit, reproduce, broadcast, publish, distribute and/or otherwise use my name (first name and first initial of last name only and any comments provided in my responses to the above survey questions (collectively, the “Testimonial”), in whole or in part, paraphrased, amplified, and/or shortened, in any and all media now known or hereinafter invented, for any legal purpose whatsoever, including for purposes of advertising, publicizing and/or promoting Jackson Hewitt. 

I represent that the Testimonial contains my own opinion based on my own experience(s), and is true and correct to the best of my knowledge and that I made the testimonial without any prior payment or promise of payment. I agree that I will not hold the Licensees responsible for any claim or liability resulting from the use of my Testimonial in accordance with the terms hereof. I certify that I am of legal age and have the right to make this agreement and I am not prohibited or otherwise unable to grant the right described above.
